Exchange Students Application - Instructions
A COMPLETE Exchange Program application should be submitted online by application deadline. A complete application includes the following items:
To be submitted BY DEADLINE:
- Exchange Students SEVIS Form
- Certification of Financial Responsibility
- Non-Degree Registration Request Form
- List of classes the student hopes to take at UF
(not required for Law students)- Copy of most recent transcript
- Academic Report Request Form
- Weaver International House Interest Form if the student wishes on-campus housing
The next two items should be submitted within 30 days after the application deadline:
There are no additional requirements to be accepted to study here on exchange: PLEASE do not apply for admission to UF, nor take the TOEFL nor submit letters of recommendation.
To study as an exchange non-degree-seeking student, UFIC will prepare a DS2019 form. The DS-2019 form is required to apply for a J visa to study in the US as a non-degree-seeking student.
The online forms are “fillable”. Type on the forms and then print out the completed form. Difficulty in reading handwriting can create mistakes and delay the processing of application material. Remember:
- in the US, all dates are written MONTH/DAY/YEAR; be sure to follow American style in writing all dates on all application materials.
- Leave blank all places where a Social Security Number or UFID number is requested UNLESS the student does in fact have a real US Social Security Number. In that case, the US social security number should be written in any space where it is requested. Students who do NOT have a US social security number should not use any number from their home country or their home institution.
A UF identification number (UFID) will be assigned to each student later.
FORMS:
- the full name, (last name, first name and middle name) must be listed EXACTLY as it appears on the student's passport.
- Please indicate a subject area that you intend to study here at UF.
- The co-ordinator at the home school must complete all the information and sign and date in the appropriate space on the bottom of p.2.
Certification of Financial Responsibility:
- You must verify that there is a minimum of USD $1000.00 for each month you will be here in the US. Amounts must be listed in the appropriate space on the form, and must be in US Dollars.
- If the bank account is in the student's name, then all that is necessary is for the bank to complete and sign at the bottom of page 3. (*)
- If the account is in the name of anyone other than the student, that person must complete the "Parent or Sponsor" part of page 3 AND the bank must complete the bottom of page 3. (*)
- For funding of any sort other than personal, an official letter on appropriate letterhead must be supplied with the application, verifying in English and with amounts listed in US dollars, that the amount listed on page 3 of the application is available.
- The student must sign and date on the bottom of page 3. WE CANNOT ACCEPT APPLICATIONS WITHOUT COMPLETE FINANCIAL VERIFICATION.
(*) If your bank will not complete the bottom of page 3, you must submit an official letter from the bank on appropriate letterhead, verifying in English and with amounts listed in US dollars, that the amount listed on page 3 of the application is available. The letter must be signed by a bank offical and dated. Send the letter with your completed application.
Non-Degree Registration Request Form:
- must be signed and dated by the student at the bottom of p.1 of the actual form (not the instruction page) and at the top of p.2 of the form.
- No one other than the student should sign the form.
- Complete all the requested information and answer all the questions.
List of classes the student hopes to take at UF:
- Students planning to take classes in the Warrington College of Business must consult the following sites for Graduate Exchange Students site or Incoming Undergraduate International Exchange Students site. In both cases, students must complete and submit the on-line course request form to the College of Business.
- Law students should consult Ms Michelle Ocepek (ocepek@law.ufl.edu) for course information.
- For all other subjects, students must submit the Course Request form to UFIC. For a list of all the classes taught in a given semester, please consult the Registrar’s website http://www.registrar.ufl.edu/soc/; click "Course Listing" for the term you are comming to UF in the schedule of courses page to look for the courses of your interest . The class periods are listed below on that page: Class Periods Fall/Spring and Summer.
- The list of classes for Fall is available by early March, and the list for Spring is available by early October.
- Classes numbered 1000-4999 are undergraduate level.
- Classes numbered 5000 and above are graduate level.
- Undergraduate students are not normally allowed to take classes at the graduate level, though permission may be given by the instructor for the course.
- Graduate-level classes higher than 6999 are not normally open to exchange students.
- Undergraduate students must take at least 12 UF credit hours each semester, and graduate students must take at least 9 UF credit hours each semester.
- We do not recommend that incoming exchange students take the maximum number of credit hours.
- The number of credits for each class is listed on the website.
- Prerequisites and a brief description of each class can be seen by clicking on the course letter-number sequence in the far-left column of the listings of courses offered in each department.
- Many classes have prerequisites.
- Each academic department decides whether it will waive prerequisite classes for exchange students.
- Students who have not taken prerequisites at their home school should not assume that they will be able to take the advanced class here.
- Not all courses are open to exchange students.
- Some classes are restricted to students majoring in the subject area.
- Some very popular classes will not have any spaces by the time incoming exchange students are able to register.
- Students wishing to take classes taught in the College of Journalism will have a very limited range of classes available to them.
- Pay attention to the level of the classes you choose, to the prerequisite requirements, and choose many alternate classes in case your first choices are not available.
- This form is required to give permission to our Registrar to prepare and send to your school one free official copy of your academic record (courses, credits and final grades; this is sometimes called a “transcript”).
- If you also wish a copy for yourself, you will have to complete an online transcript request form before leaving, and pay the regular student charge for a transcript.
Weaver International House Interest Form if the student wishes on-campus housing:
- On-campus accommodation is available in Weaver International House. Most rooms are shared with UF degree-seeking students, though there are a few (more expensive) single rooms available. Space is limited, and is allotted on a first-come, first-serve basis, so students wanting to live in Weaver House should submit the Interest form as soon as possible.
Mandatory Immunization and Health History Form:
- All the listed requirements must be met before you can attend classes.
- Even if it is not required in your country, you must comply with all of the requirements listed on the form.
- All students MUST have had two measles shots, the first AFTER their first birthday and the second at least one month later, AND a rubella shot, or two MMR shots.
- All students MUST have had a Mantoux TB test within the PAST TWELVE MONTHS, and the date of testing and the results must be listed.
- BCG vaccine administered in childhood is NOT acceptable instead of the PPD test. The TB test must be administered and read before the measles shot, or administered on the same day as the measles shot.
- If the measles shot is given first, the student must wait 30 days before having the TB test.
- All students must EITHER have the listed Meningitis and Hepatitis B shots, OR make a check-mark beside each sentence declining the shots, AND sign and date on the line at the bottom of the WAIVERS section in the top-right-hand corner of the form.
- You will not be able to register for classes until your Immunication hold is removed
Verification of Health Insurance:
- Students will not be allowed to attend classes until they have provided verification that they have health insurance meeting all the specifications listed on the Insurance Verification form.
- They may use insurance from their home country provided that the company will complete and sign the form.
- A completed form MUST show a start date AND an end-date for coverage.
- It must list the student's name, and be signed and dated by a representative of the company.
- Students may purchase insurance from an American company if they wish, and that can even be done before they arrive, using on-line websites and a credit card.
- You will not be able to register for classes until your Insurance hold is removed.
- To remove your Insurance hold, please forward your insurance receipt /confirmation, or Insurance Verification to insurance@ufic.ufl.edu. Holds will be removed within 24 hours.
