Your health is important to us and we want you to stay well during your stay in Florida. Our law also requires you to meet Florida state immunization regulations. Without proper immunization, you will not be permitted to register for or attend courses at UF.
To verify your vaccinations, your doctor must complete the Mandatory Immunization and Health History Form, provide the dates of your immunizations and injections, sign, stamp, and date the form.
- Tuberculosis test is REQUIRED of all international students. The TB test must have occurred within the last year. The doctor must indicate the date and result of the test. NOTE: If test result is positive, a chest x-ray must be performed and the doctor's report of the x-ray results must be submitted with the Immunization and Health History Form.
NOTE: BCG vaccine administered in childhood is NOT acceptable instead of the PPD test. The TB test must be administered and read before the measles shot, or administered on the same day as the measles shot.
- Meningitis and Hepatitis B vaccines are not required. If you do not have not these immunizations, you must make a check-mark on the line beside each sentence saying you decline the vaccines and you must sign and date the waiver form on the lines provided.
If the UF Student Health Care Center does not approve your immunization form, you will be required to have the necessary vaccinations when you arrive, at a cost of $40 or more.
Please read carefully the instructions for the Mandatory Immunization and Health History Form. Use only BLACK ink to fill in the form.
Your UFID number must be written on the space provided on the Mandatory Immunization and Health History. The UF Student Health Care Center will not accept forms without a UFID.
Students should include a valid email address on the Immunization and Health History form so that they may be contacted if needed.
